Medical Billing Checklist for New Practices: The First 90 Days
Opening a new practice? Use this step-by-step billing checklist – from NPI and credentialing to your first clean claims – to get paid from day one.
Opening a new practice is exciting, but billing mistakes in the first months can delay payments for a long time. Use this checklist to set up your revenue cycle the right way from day one.
Before you see your first patient
- NPI numbers: an individual NPI for each provider and a group NPI for the practice.
- Tax ID and business setup: make sure the legal name and address match on every application.
- CAQH ProView profile: complete, attested and shared with the payers you plan to join.
- Credentialing and payer enrollment: start early – Medicare, Medicaid and commercial payers can take weeks to months.
- Fee schedule: set your charges for the codes you will use most.
- EHR / practice-management system and clearinghouse: set up electronic claims and electronic remittance (ERA) and direct deposit (EFT).
Your front-desk workflow
- Verify insurance eligibility and benefits before every visit.
- Collect copays and deductibles at check-in.
- Request prior authorizations ahead of services that need them.
- Scan insurance cards and IDs into the patient record.
Your first 90 days of billing
- Submit claims within 24–48 hours of each visit.
- Work every rejection the day it comes back.
- Post payments and review each remittance for underpayments.
- Follow up on unpaid claims after 30 days.
- Review a simple monthly report: charges, collections, denials and A/R by age.
