How Much Does Medical Billing Cost? Pricing Models Explained
Percentage of collections, flat monthly fee or per-claim pricing – how medical billing services are priced and what drives the cost for your practice.
One of the first questions practices ask before outsourcing is simple: how much will medical billing cost? The honest answer is that it depends on your specialty, claim volume and the services you need. This guide explains the common pricing models so you can compare proposals with confidence.
The three common pricing models
1. Percentage of collections
The most common model. The billing company keeps a small percentage of the money it actually collects for you. Because its income depends on your collections, it is motivated to submit clean claims and follow up on every denial. Industry rates generally fall in the low-to-high single digits, depending on specialty and volume.
2. Flat monthly fee
A fixed amount each month, usually based on the number of providers and expected claim volume. It makes budgeting easy and suits practices with steady, predictable volume.
3. Per-claim or per-service pricing
You pay for each claim submitted or for individual services such as credentialing, eligibility checks or A/R recovery. This works well if you only need part of the revenue cycle handled.
What affects the price?
- Specialty: specialties with complex coding, modifiers or prior authorizations need more work.
- Claim volume: higher volume usually lowers the effective rate.
- Number of providers and locations: each provider must be credentialed and billed correctly.
- Services included: full revenue cycle management costs more than claim submission alone.
- Existing A/R: older unpaid claims take extra effort to recover.
Watch for hidden costs
Ask every billing company for a written proposal that lists setup fees, software fees, minimum monthly charges and cancellation terms. A low headline rate can become expensive if denials are not worked or if extra services are billed separately.
